What insurance can you accept?

We can all accept Motor Vehicle Accidents/Claims (MVAs).  The state of Oregon requires drivers to have a minimum of $15,000 in personal injury protection for injuries sustained to them or their passengers in a motor vehicle accident.  In addition, we are in-network with the following companies.  We are not registered with the state and are unable to accept OHP:

I thought I paid you while I was there, why did I receive a bill after my appointment?

When we are billing an insurance carrier on your behalf, you may only be required to pay a fixed co-pay at the time of your appointment.  You are charged at your appointment based on the information we received from your insurance company about your coverage, which is not a guarantee of payment.  You may be sent a bill if your insurance company determines that you owe toward your deductible, additional co-insurance, or if there is a non-covered service or amount.  Elemental Medicine does not determine how much you owe or why, this is determined solely by your insurance carrier and explained on your EOB (Explanation of Benefits).

What are deductibles, co-insurances, and non-covered services?

Your deductible is the amount you must pay, usually annually, toward your health care before your insurance company pays for anything.  As an example, if you have a $250 deductible that has not been met and you receive $200 of covered care from our office, you will owe our office $200 after your insurance company processes your bills.

A co-insurance is a percentage due on the covered amount of your medical services, often in addition to a fixed co-pay.  For example, if you have a 20% co-insurance and you receive $100 in covered services, you will owe $20 for those services.

Non-covered services or amounts are determined by the coverage of your specific policy.  Some companies do not pay for the additional therapies that Dr. Ebling may use as part of a chiropractic treatment, such as massage or home exercise instruction.  These are considered non-covered services.  Often policies have a limit on number of treatments or dollar amount of care per year.  For example, if your policy allows for 10 treatments per year and you receive 11, the 11th visit will be denied by your insurance carrier as a non-covered amount.  You will receive a bill for these non-covered services or amounts
